What to expect
Secure intake forms
Your information will be entered into our HIPAA-secure EMR system, EDVAK. You will then receive an email and text with a link to the required intake forms. All you need to do is submit the forms and photos — and you're done!
Payment after your visit
You will be directed to enter your payment information, and your payment will be processed after your visit is complete.
Physician review within 48 hours
After you complete the forms, please allow 48 hours for one of our physicians to review your chart and respond with a recommended treatment plan.
Prescriptions & treatment plan
If prescriptions are required, we will send them to your pharmacy of choice based on your intake form, or to one of our pharmacy partners for compounded medications — who will reach out to you for shipping and payment information. Our doctors will also provide detailed instructions on how to use your prescribed medications.
